Should you want to replace your designated caregiver, our care coordinators will work with you to help you select the most compatible caregiver for you.
Step 4: During the delivery of services, our care team will conduct regular follow-ups and monitoring, whether through a phone call, video chat, or home visits, to oversee your response to care and current condition. This will then allow our team to create modifications to your service plan when deemed necessary.
